The Custom Online Application Disclosures report allows users to identify anyone who has checked the custom disclosure boxes along with their contact information. The report includes App Number, School, Disclosure, Student Name, Contact Name, Address (City, State, Zip), Email, Phone, and Disclosure Selected (true or false).
This report includes a date and time stamp in the footer to show when the report was generated. The time reflects the Central Office time zone.
Report Criteria
You can specify the following criteria for running the report:
•Export to the screen or to a PDF file.
•Specify a date range.
•Run the report for Schools or School Groups.
•Filter by School/School Group, Grade, and Disclosure.
