Districts using the Household Survey feature can generate a Household Survey Information Status report to reference student household information status based on the survey results (household size and income range).
The report includes:
• Student’s last name, first name, student ID, and school
• HHS# (generated during the household submission at the website)
• Submission Date (date the household survey was submitted)
• Household Size and Income Range (selected at the household survey website)
• Case Number (household member case number)
• HHS Info Status (Free, Reduced, Full Pay)
• Signer Name (household survey signer at the website)
You can specify the following criteria for running the report:
• Export to the screen, PDF, CSV (comma delimited), Excel, Rich Text Format, TIFF file, or Web Archive.
• Run the report for Schools or School Groups.
• Filter by School/School Group and Month/Year.
• Specify a date range.
• Specify a Household Survey Information Status (ALL, Free, Reduced, or Full Pay).
