Household Survey

Family income data is not collected through the free and reduced-price meals application form in CEP schools. The Household Survey feature provides CEP administrators with a method for collecting data to determine percentages of economically disadvantaged students for state funded programs that require this information for funding purposes.

Note: After a user acquires an Online Household Survey (HHSurvey) license, in order to be able to see the Online Household Survey option under Admin > Definitions/Setup > Community > Eligibility, the user must access User Groups, enable the Online Household Survey option under Group Rights, log out, and then log back in.

To enable the Online Household Survey feature, check the Enable Household Survey Website checkbox (Admin > Definitions/Setup > Community > Eligibility > Online Household Survey).

Horizon School Technology Eligibility

The Welcome to the Online Household Survey page provides information about the purpose of the survey—it is used to determine eligibility for state benefits for which the children of the parent/guardian may qualify. Only one survey is required for all the children that attend schools in the same school district.

Household Survey Students

The parent/guardian is prompted to enter the student’s First Name, Last Name, Date of birth, Grade, and School.

A This child is a… section appears under the Date of birth fields. The options that display (Foster, Homeless, Migrant, Runaway) are set on the Online Household Survey page (Admin > Definitions/Setup > Community > Eligibility > Online Household Survey).

The parent/guardian can optionally indicate the ethnicity and race of the student.

Household Survey Assistance Programs

The parent/guardian is prompted to indicate whether any household member is enrolled in assistance programs. The programs that appear on this page are set on the Online Household Survey page (Admin > Definitions/Setup > Community > Eligibility > Online Household Survey).

Household Survey Household Members

If there are other children in the household, the parent/guardian can add the child’s information on this page. If there are other adults in the household (other than the parent/guardian), the adult information can be added.

Household Survey Income Range

The parent/guardian is prompted to select the income range that applies to their household. The values are based on the USDA Income Eligibility Guidelines. The survey does not affect or determine a child’s school meal benefits; it is used to assist the school/district in measuring and supporting student success.

Household Survey Contact Information

The parent/guardian is prompted to enter the household address and primary contact phone and email information in case they need to be contacted regarding their survey results.

Household Survey Review

The parent/guardian can review and edit, if necessary, the information they provided during the survey (students, adults, assistance programs, household income, contact information) before they sign and submit the survey.

Household Survey Submitted

A system-generated survey number will appear on the Household Survey Submitted page (the page can be printed for reference).

The survey will be reviewed by the local school district official.
